Absolutely filthy. Dirt, dust, muck, who knows what on the chairs, tables, and floors so thick you can barely see clean surface. The toys are even worse. I only went to get a family member's child and it honestly would be comparable to walking into a large dumpster. I had to clean my kid's hands with baby wipes after ten minutes of being there. No cleanliness standards. No extra precautions when there's an obviously very ill child there.
