My 95 year old mother was there for 51 days. She needed some medical attention for a wound that got infected on her foot. She was seen by the nurse and bandage changed regularly. Little delay with getting her antibiotics, but that did occur. She saw the Dr weekly. The facility is clean and well lit. When I visited a couple times a week, I always saw cleaners with their carts in the hallways cleaning all areas of the facility. Her room, bed and bathroom were always clean. My mother liked most of the food and did eat regularly, which was not the case at home prior to coming to this facility. Although, she hated the coffee. There are activities that my mother liked and participated in. She had alot of fun at the activities. She had her hair done at the on-site salon, after getting an appointment. The management staff was kind and knowledgeable. They were in contact with me on a regular basis, even when there was a Covid outbreak. They called me to let me know the details and everyone was wearing a mask the next visit we made, after their call. Management seems new, but all have years of experience. We met with all of them when my mother was admitted. The nurses were professional and available. Nurses Aids were not in abundance and some room calls went unanswered for well over 15 minutes. Their beds do not have side rails and my mother slipped out of the bed and onto the floor one evening when taking herself to the bathroom. But most of all she was comfortable there and even said that if she had to go back to another rehab facility that has medical treatment, that she would return there. Shout out to Aubre who was always very helpful and available when we had questions.

I wouldn't give this place one star if I could. After reading some of the other reviews I gave the place the benefit of doubt. This is the only place that was open at the time for my husband to be transferred to from the hospital. I felt I needed to be there every day because he needed help to be fed, always needed water which they never gave him. He was cold. Left alone. He had severe bed sores which became infected. He was not secure in his bed or wheelchair... both he fell out of. There were only a chosen few who really did help him. The higher uppers told you what you wanted to hear but didn't follow through. Thanks to Aubrey for her help. I would never recommend anyone to stay here.

I wouldn't put the worst of the worst in this home unless you were guaranteed to be cared for by Jazz, Billie, Hunter, Allison, Rick and a handful of others. My mother was in there for 2 weeks with the intention of not needing skilled nursing when she came home after being in the hospital since June 16th. They drew blood and never got it picked up, after the "lead" nurse refused to draw blood in front of an audience. Her words. A head nurse. Fine. We left the room. All for "stat" blood work to be drawn 4 hours after the order went in to never be picked up. Blaming quest for not picking up and order they never received. Because yes, I contacted quest to ensure an order to pick up was received. They then said a meeting with all heads of departments was needed to allow the family to understand her treatment plan. The therapists director showed up, the social worker (who lies and covers up her mistakes daily) and an insurance liason if you will who had nothing to add to the meeting. The head nurse would not even come up stairs. How do I know? Be cause I heard the conversation between her and Ariel. The doctor claims he was giving information to be passed on and we never would receive it. They would call and not leave messages leading you to believe an emergency was happening and then tell your other loved ones that they could never get through and couldn't leave messages. Meanwhile you could be sitting in the room with your loved one and they won't even enter the room. You hit the nurse call bell and wait sometimes for hours. I watched patients asking nurses what meds they were getting and the nurse not be able to tell them even what they were administering. I heard a male aide refuse to take a patient to the bathroom because he had other things to do. His words. Can't be misconstrued when they come directly out of their own mouth. Was told at 330 on a Friday I had to have my mom out on Monday. No discussion to make sure I had everything she needed. No discussion about treatment at home or patient appointments for follow ups. Talk to Aetna and they said they were told she was 100% better and could do everything on her own. She needs help just standing up. This establishment needs management that shows responsibility and actually does their job. You call down to the facility and it will ring and ring and ring. When and if you get someone they then transfer you to a nurses station that no one answers. Leave messages to never be returned but you don't answer your phone. I could ramble on and on. Terrible facility. The Aides though ... Those girls are amazing. AMAZING. When I was falling apart it was Jazz who made sure I was ok as well as my mom. Not one nurse. Not one director. The remaining facility is a joke but Jazz and her friendly people on her floor, amazing.

If I could, I would give 0 stars! This place is terrible! My MIL went in for a short rehab stay on the Friday prior to Memorial Day, being able to walk short distances....by Tuesday, she had a rapid decline. They wouldn't raise her bed, told her to eat laying flat. Told her to use the bathroom in the bed and they'd clean her up when they got to her. I called the facility after my FIL and husband went to visit, asking about a transfer to the ER. The nurse supervisor said "there is no reason for transfer".....I gave them three minutes to call emergency transport or I would, which would look terrible if family called. They called and had her transferred back to Forbes ER. She arrived to the ER with a high fever, bed sores & pneumonia. They also didn't give her medication dosages properly. This place should be shut down, they shouldn't care for anyone or anything. They went to visit my MIL at the hospital, trying to ask her questions when she wasn't in her right thought. Since then, I have talked to numerous people from regional and district offices trying to cover themselves because they know that they are in trouble. Unless you don't want your loved one to get better, don't send them here because they will not care for your family right.

my wife had rapid decline in my care. A lady posed as a nurse stole my pain medication. She was in tremendous pain. Had she stayed there she could have died
